Welcome to End of Life Coalition
Mission — Increase access to end-of-life services by eliminating barriers, enhancing services and reaching out to the community.
Vision — To educate individuals and caregivers about choices for end-of-life care and provide them the tools and resources to implement those choices.
Values — The coalition conducts its services in a professional manner with integrity and promotes the value of life across the continuum of care, encouraging self-determination within choices provided.
The coalition meets from 8 to 9 a.m. on the last Friday of each month in the Memorial Health Care Conference Room. Members include hospice professionals, social workers, registered nurses, aging agency directors and others who share the coalition's commitment to further its mission in the community. The coalition is a member of the Nebraska Hospice and Palliative Care Partnership.
The coalition is active in Seward County educating residents about end-of-life issues in community settings, medical facilities, senior centers and workplaces. The coalition actively promotes National Health Care Decision Day that emphasizes the importance of establishing an advanced directive.
The coalition is funded by grants, and corporate and private donations.
We encourage all community members to visit a monthly meeting to learn more about us.

End of Life Coalition Announces its 2012–2013 Community Scholarship
End of Life Coalition $500 Community Scholarship
This scholarship was designed for students residing in Seward County who will be in college during the 2012-2013 school year. This $500 scholarship will be awarded to a qualified applicant who has reached out to a family or community member dealing with a chronic illness.
Requirements
Applicants are required to present a maximum 750-word essay describing his/her experience of reaching out to a family or community member dealing with a chronic illness. Include the following components:

Explain how this experience impacted your life

What you gained/gave from this experience

How you plan to apply this experience to your career
